How to use the Nursing Reference Center (see link below) to find care plans (care sheets):

Select the Diseases & Conditions tab
Type the medical condition in the text box, e.g. hepatitis
Select "Evidence-Based Care Sheets" from the option on the right
Select Browse

Point-of-Care resources include:
Collection of evidence-based care sheets and patient handouts, disease information, its signs and symptoms, typical diagnostic tests and interventions, cultural competency guidelines, point-of-care drug information, continuing education modules, nursing skills and procedures guidelines, skill competency checklists, books for nursing reference, legal cases, research instruments and latest medical news.

How to use CINAHL (see link below) to find care plans:

Type the medical condition in the text box, e.g. hepatitis
Scroll down the page and find Publication Type
Within the Publication Type limiter, select Care Plan
Scroll down the page and select Search

Scholarly/peer-reviewed articles with full-text for 1,400+ and indexing for 5,400+ journals. Also includes evidence-based care sheets, continuing education units, magazine articles, books dissertations, selected conference proceedings, standards of professional practice, educational software and audiovisual materials. CINAHL provides a Cited References Search screen. Coverage: 1937 – present.

The Library Catalog may be used to identify books and ebooks that contain nursing care plans:

Search the catalog using the keywords nursing care plans.
- books - you will need the call number for titles located in the Main Stacks.
